Scheduling Oral Surgery Appointments
We ask that you call us at Peninsula Oral Surgery & Implants Office Phone Number 310-325-7800 to schedule a consultation prior to surgery. Your initial appointment will consist of a comprehensive assessment of your medical health history and a consultation explaining your diagnosis and treatment options. Additional x-rays may be taken in an effort to fully understand your clinical situation. Once a treatment plan is decided, our financial coordinator will assist you with an explanation fees.
Office Hours
Please provide the following information at the time of your consultation:
- Referral slip from your dentist
- X-rays, if applicable (or verify they have been sent to us)
- A list of medications you are presently taking and medical health history
- Dental insurance information so our treatment coordinator can determine your benefits and submit claims on your behalf
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian to all visits

Online Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. If you are unable to preregister online, please plan to arrive 30 minutes before your appointment to complete the registration in person.
